A couple of weeks ago, my tenant complained that there was too much water falling onto to the bathroom floor when taking a shower, and that this was a drowning hazard for her kids. We offered to install a proper shower screen to stop the water from coming through. The house is 50 years old, there is only one drain in the shower, but no drain on the bathroom floor. As a result of this, she is refusing the pay rent unless the second drain is installed.

First of all, is it legally required to have 2 drains in a bathroom? Especially considering the house is so old and there was always one to begin with. Any advice on how I should tackle this problem?

    Drowning hazard - seriously?
She's not allowed to withhold rent - sounds like an excuse. Issue a breach notice for non-payment of rent.
Lots of older homes do not have a floor waste in bathrooms or laundries. It's not a requirement to retrofit either.

The hilarious part was that she had an unfenced inflatable pool which compliance reported to me last month..yet water in bathroom was more dangerous

Why don't you have a chat with the local council, and see if they need to pay her a visit...
