    indeed it seems a case of shocking culture

    ( disclosure i hold AMP )

    there is always the risk of human nature related outcomes , but in some companies the checks , balance and oversights seem to fail completely ( and for up to a decade )

    will such trends be fixed this time ,
    after all this inquiry was a white-wash turned into witch-hunt and has so far performed less spectacularly in both aims ( much to the average citizen's benefit )
